Here’s what to expect next:

  • Check Your Email Inbox/Spam folder
    • You’ll receive a confirmation email shortly with the details of your appointment.
    • This email will include: Your appointment date and time and a link to the patient portal to complete your intake form online.
  • Complete Your Intake Form
    • Please fill out the intake form as soon as possible so we can better understand your health goals and medical history and send you your lab order.
    • If you are using insurance for labs please upload a copy to the portal.
  • Get Your Labs Done
    • Take your lab order to your preferred lab location and have your tests completed at least 2 days before your appointment.
  • Prepare for Your Visit
    • Bring any questions or concerns to your appointment. We’ll discuss your lab results, medical history and symptoms,  and create a personalized care plan designed just for you.
    • For virtual visits, you’ll receive a link in your confirmation email as well as at the time of your visit.

Cancellation Policy: 

As a courtesy to our patients and providers, please provide adequate notice (24 business hours for follow up appointments and 48 business hours for new patient appointments) if you need to cancel or change an appointment.

Cancellations without adequate notice or no-shows will be subject to a missed appointment fee of $50.00 for follow-up appointments and 100% of fee for new patient appointments or forfeiture of the prepaid service. Any accrued fee balances must be settled prior to scheduling new visits If you are more than 10 minutes late for your appointment, please let us know, you may have to reschedule. Thank you.

  • If this is your first time logging in to the patient portal, please click on the ‘New Sign Up’ button to set your initial password.
  • A link will be sent to this email that will allow you to create a password and then sign on to the portal.
  • The portal will provide you access to your handouts, provider messaging treatment plans, billing information, and more; you will also be able to request future appointments through the portal.
  • Please note, we cannot order your labs until the demographic information is complete.
