If you have a side effect from a medication
Stop the medication and call us. If the reaction is severe, call 911. Please tell us about any reaction so we can record it in your chart, notify the pharmacy and, where required, report it to the FDA. You can also report a side effect yourself at FDA MedWatch or 1-800-FDA-1088.
If you have a complaint
Tell us first. The practice owner reviews every complaint personally and replies as quickly as possible, usually within five business days. You also have the right to contact the Virginia Board of Nursing (804-367-4515, dhp.virginia.gov/nursing) about a provider, the Virginia Board of Pharmacy (804-367-4456) about a pharmacy, or the U.S. Department of Health and Human Services Office for Civil Rights about privacy (see our Notice of Privacy Practices). We will never retaliate against you for raising a concern.
