Testosterone replacement therapy · Richmond & Midlothian, VA

TRT that starts with answers, not a prescription.

Low energy, low drive, and strength that’s harder to keep, while routine labs say everything is fine. We test free and total testosterone, estrogen balance, thyroid, and metabolic function before recommending anything, then build testosterone therapy around your results.

The evaluation is the first step for our hormone, weight, and longevity programs. Looking for everyday primary care? Direct Primary Care members enroll online, no evaluation needed.

Signs of low testosterone

Most men aren’t looking for a prescription. They’re looking for an explanation.

Fatigue that doesn’t respond to sleep. Strength that’s harder to maintain despite the effort. Brain fog that affects work, focus, and drive. Weight that won’t move no matter what you do. These symptoms are real, and they’re rarely caused by one thing.

Fatigue, brain fog, low drive, and body composition changes can come from testosterone, thyroid dysfunction, metabolic imbalance, or a combination. That’s why we evaluate the full picture before drawing conclusions.

Men often come in with

  • Persistent fatigue or low energy
  • Difficulty building or maintaining muscle
  • Weight gain or changes in body composition
  • Brain fog, low focus, or mental fatigue
  • Reduced libido or erectile dysfunction
  • Poor sleep or non-restorative rest
  • Low motivation, confidence, or drive
  • Mood changes, irritability, or flat affect
Why “normal” labs miss it

Total testosterone is one number.

Standard panels check total testosterone. That doesn’t tell you how much is biologically active, how your estrogen balance is affecting symptoms, whether your thyroid is contributing, or how inflammation and insulin resistance are working against you.

Many men are told their levels are “normal” while experiencing every symptom of hormonal decline. Normal by a lab reference range is not the same as optimal for how you function.

What we evaluate

  • Free and total testosterone
  • Estrogen balance
  • Thyroid markers
  • Metabolic function, including insulin resistance
  • Inflammation and lifestyle factors

The answer is rarely in a single number.

How TRT works here

The Venture Wellness Men’s Care Framework

Structured, ongoing, and built around your biology, not a one-size protocol.

  1. Stabilize

Comprehensive intake and lab evaluation to establish a safe, effective TRT starting point.

  2. Optimize

Testosterone replacement therapy tailored to your symptoms, labs, and goals.

  3. Maintain

Ongoing follow-up, lab monitoring, and dose refinement to keep you feeling your best.

  4. Evolve

Care adapts as your body, training, stress levels, and life stage change.

What to expect

  • Your plan is personalized based on findings, not selected from a standard tier
  • Dosing is titrated based on your response and monitored through regular labs
  • You have direct access to your provider throughout your care, not a receptionist hotline
  • Care runs on a quarterly basis, structured around the biological rhythms of hormone and metabolic health
  • Testosterone is often the cornerstone of care, with additional support layered in to improve outcomes: thyroid, metabolic and weight support, peptides, NAD+, or erectile dysfunction treatment when your findings support it
What TRT costs

Clear pricing, reviewed with you before you continue.

Everyone starts with a new patient evaluation: an unrushed visit, a full review of your results, and a personalized plan recommendation covering hormones, thyroid, metabolism, and any other relevant findings. You leave with clarity, not a follow-up to schedule a follow-up.

Costs at a glance

  • New patient evaluation: $155
  • Comprehensive lab panel: billed to your insurance by the lab, or $195 self-pay
  • Ongoing care: quarterly, starting as low as $160 a month, covering lab monitoring, dosing adjustments, and proactive check-ins

Your plan and its cost are reviewed with you before you continue. All care is individualized and provided by a licensed medical provider. Results vary based on individual factors.

Worth the drive

Men come to us from across central Virginia.

Our office is in Midlothian. Patients drive in from Short Pump, Glen Allen, and the West End, downtown Richmond, Chesterfield, Chester, and Powhatan, and from smaller communities across central Virginia that don’t have a men’s health clinic of their own. Once you’re established, follow-ups can be done by video.

Care runs quarterly rather than weekly. Once you’re an established patient, follow-up visits can be done by video. That keeps the schedule manageable from a distance.

Already on treatment?

Switching your hormone care to Venture Wellness.

Many of our patients arrive already on treatment: from a telehealth-only clinic, a practice that has closed or changed hands, a provider who never adjusted the dose, or a pellet clinic they’ve outgrown. Switching is routine for us, and there is no gap in your care.

  • Your recent labs count. Bring them, or have them sent, and we order only what’s missing rather than repeating the full panel.
  • You still start with an evaluation. Every provider manages hormone therapy differently. Your evaluation is how we learn your history, your current dose and schedule, and how you’ve responded, so what we continue or change is based on you, not on a chart from somewhere else.
  • Keep your current treatment as prescribed until we’ve reviewed it together. Don’t stop or change anything on your own before the visit.

Coming off pellets

We transition patients off pellets regularly. Pellets release hormone gradually, so the best time to make the change is close to when your next insertion would be due. Book your evaluation a few weeks ahead of that date so your new plan is ready when the pellet wears off.

What to bring

  • Your most recent labs and the date they were drawn
  • The name, dose, and schedule of your current hormone prescription, and the date of your last dose or pellet insertion
  • Any other medications and supplements you take
  • A short note on what has and hasn’t worked for you
Questions

TRT FAQ

How do I know if low testosterone is causing my symptoms?

You may not, and that’s the point. Fatigue, brain fog, low drive, and body composition changes can come from testosterone, thyroid dysfunction, metabolic imbalance, or a combination. We evaluate the full picture before drawing conclusions. While many of our male patients benefit from testosterone replacement, we provide a more comprehensive evaluation to target symptoms unresolved by TRT.

How much does TRT cost at Venture Wellness?

The new patient evaluation is $155. The comprehensive lab panel is billed to your insurance by the lab, or $195 self-pay. Ongoing care runs quarterly and starts as low as $160 a month. Your plan and its cost are reviewed with you before you continue.

Will TRT affect my fertility?

Testosterone replacement suppresses the body’s natural production of sperm. If fertility preservation is a priority, we discuss options, including alternatives that support hormone optimization without suppressing sperm production. This is part of the evaluation conversation. We also offer sperm testing and banking for patients who have fertility concerns.

How long until I feel results?

Early improvements in energy and mood are often noticed within 3–4 weeks. Full benefits, including body composition, strength, libido, and cognitive clarity, typically develop over 3–6 months as your protocol is optimized. Results vary from person to person.

What does ongoing care look like after my first visit?

Care runs quarterly, structured around lab monitoring, dosing adjustments, and proactive check-ins. You’re not left to manage things on your own between visits, and you have direct access to your provider throughout your care. Once you’re an established patient, follow-up visits can be done by video.

What other treatments might be part of my plan besides TRT?

Depending on your evaluation and health goals, your plan may include supplement recommendations, thyroid support, metabolic and cardiovascular health support, GLP-1 therapy, peptides for recovery and performance, NAD+ for energy and cognition, erectile dysfunction treatment, or joint and soft tissue support. Interventions are added one at a time as medically appropriate.

I’ve had a wonderful experience with Dr. Julia Montgomery. From the beginning, she took the time to listen to my concerns and develop a personalized plan focused on helping me feel like myself again. I appreciate how comfortable and supported they make me feel during every appointment. They are responsive to my feedback, genuinely care about how I’m feeling, and are willing to adjust my treatment plan based on my symptoms and progress. Rather than taking a one-size-fits-all approach, they work collaboratively with me to find what works best for my body. I’m grateful for their knowledge, compassion, and commitment to helping me improve my overall well-being. I highly recommend them to anyone looking for a provider who truly listens and partners with you on your health journey.

I am so thankful that I waa able to recently become a patient at Venture Wellness! Dr. Julia and her assistant Maria are so patient, helpful, informative and easy to talk to. I am 50 years old and have not been feeling like myself for quite some time. I knew hormone changes were taking place but I wasn't sure where to turn. Dr. Julia has helped me get back to feeling like myself again, and in some ways, even better. My brain fog is virtually gone, focus is back, energy has returned and I just overall feel so much better! Maria has been a super help as well making sure my labs are done when they are needed and she has helped me several times when I have required additional assistance with questions and support. I am also working with Venture on a small weight loss journey to get back to where I hope to be and with their help I am seeing steady results. In addition to all of the improvement I have seen with my health I also should acknowledge the depth of information about my health that I have learned from Dr. Julia that had never been reviewed with me in the past with my primary. I cannot recommend this practice enough if you are feeling off and want some help so you can feel like yourself again!

Working on hormone issues with Julia. She is kind, compassionate and very thorough. I tell my friends about her services, as hormone replacement therapy is still widely misunderstood and/or treated. Highly recommend her and her team!
